Behavioral Health Assessment

My  Patient Center provides comprehensive, interactive evaluations.
Please do a "Behavioral Health Assessment" when prompted.
This is extremely helpful in our first session.
A "Review of Systems" is also helpful to evaluate your general medical condition.

About appointments

How can I schedule an appointment with you?

 

            The most reliable way to schedule your next appointment with me is in person before you leave the office after that day's session.  This reduces the time and resource intensive "appointment phone tag".

 

            By calling my office and following the voice prompts, you'll either be connected to my assistant or transferred to her voice mail.  When leaving a message, give us specific days and times when you will be available as well as a voicemail enabled phone number so that we can inform you of your appointment time whether you are available are not.  This will reduce unnecessary phone tag which could delay your appointment.

 

            You may also make an appointment request by visiting my online office accessible through my OnLine Office Page.  After registering, you'll be prompted to select times and days when you are available.  We will inform you of your appointment via e-mail.


How long will my appointment be?

 

            My standard medication management visit is 15 minutes.  Longer appointments may be scheduled if you and I deem it clinically necessary and your insurance company approves that visit.

About medication refills

At the end of each appointment, I will tell you when I would like to see you for your next follow up appointment.  I will also provide you with enough refills to last you until that appointment.. If you're about to run out of refills, this usually means it’s time for an appointment.

Telephone based refills are among the error prone procedures in medical practice. As such, as a matter of policy, we do not accept phone request for refills, nor will we phone a refill to a pharmacy. Instead, we ask you to have your pharmacist request refills via secure fax. Our refill fax number is 847.701.0431 although most pharmacies have this number in their database.

You may also request electronically submitted refills through my OnLine Office.

Coordinating your care

Quality care requires communication between members of your health care team.

 Accordingly, we strive to share key clinical information with your primary care physician as and any therapists treating you. If you allow us to release information and provide us with their phone and fax numbers, I can provide your doctors and therapists with updates on our work together immediately after our session.

You may download release of information forms in advance.  Be sure to include your doctor's fax number. I have found faxes the most effective way to communicate with other members of your health care team.

About payment

The increase in costs associated with our style of practice continues to outpace the level of reimbursement.  As a result, our practice management company is collecting a small yearly fee to help us keep up with the administrative burden of letters, refills, insurance authorizations, etc

We believe that the private practice model of care is the most effective way to provide psychiatric care.

Likewise, we ask you to pay your bills in a timely fashion.  You must pay your co pay when you se me.   On your behalf as well as ours, we will not allow you to build up large balances
